Zepbound and alcohol: what do the label and the research say?

Short answer

The Zepbound label and Medication Guide say nothing directly about drinking alcohol. Alcohol appears only as an inactive ingredient and in the alcohol swab steps. The label does warn about low blood sugar, pancreatitis and dehydration, and alcohol can bear on the first two. Whether and how much to drink on Zepbound is a question for your prescriber.

What the label says, and what it leaves out

The Zepbound prescribing information and Medication Guide, both revised in August 2026, give no advice about drinking alcohol, and the drug interactions section does not list it. The word appears in two places only. Benzyl alcohol is an inactive ingredient in the multi-dose vial and the single-patient-use KwikPen. The Instructions for Use for the vial and KwikPen tell you to wipe the stopper or seal with an alcohol swab. So there is no label rule for or against a drink. That silence is not a finding that alcohol is safe with Zepbound. The question was simply not addressed.

Three label warnings where alcohol is relevant

Low blood sugar. The label says Zepbound lowers blood glucose and can cause hypoglycemia, with a higher risk alongside insulin or a sulfonylurea. In its guidance for people with diabetes, the NIDDK says alcohol makes it harder to keep blood glucose steady and can hide the first symptoms of a low.

Pancreatitis. The label warns of acute pancreatitis, and the NIDDK lists heavy alcohol use among the most common causes. Dehydration. The Medication Guide says diarrhea, nausea and vomiting can cause fluid loss that may lead to kidney problems.

Does Zepbound reduce drinking?

Zepbound is not approved for that, and we could not find a published randomized trial of tirzepatide for alcohol use. The closest evidence is for semaglutide, a related drug. In a 2025 trial of 48 adults with alcohol use disorder, 9 weeks of low-dose semaglutide reduced drinks per drinking day and craving, but not average drinks per day or the number of drinking days. A 2025 meta-analysis found that randomized trials so far showed no significant effect on alcohol consumption, while observational studies linked GLP-1 drugs with fewer alcohol-related events.

What to raise with your prescriber

Ask about alcohol before you start, and say so if you take insulin or a sulfonylurea or have had problems with your pancreas, which the Medication Guide asks you to report. The Medication Guide says to stop using Zepbound and call your provider right away if you have severe pain in your stomach area that will not go away, with or without nausea or vomiting, sometimes felt in your back. It also says to tell your provider right away about nausea, vomiting or diarrhea that does not go away.
