Is 10 mg a normal starting dose for Zepbound?

Short answer

No. The FDA label says the recommended starting dosage of Zepbound is 2.5 mg once a week for 4 weeks, then 5 mg, with further increases of 2.5 mg after at least 4 weeks on each dose. The label does not describe starting at 10 mg. Only your prescriber or pharmacist can say why your prescription differs, so call them.

What the label says

The Zepbound label, revised in August 2026, sets one starting dosage for every approved use: 2.5 mg injected once a week for 4 weeks. After 4 weeks, the dosage goes to 5 mg once a week. From there it may be increased in 2.5 mg steps, after at least 4 weeks on the current dose.

The label is specific about what the first dose is for. It says the 2.5 mg dosage is for treatment initiation and is not approved as a maintenance dosage. The recommended maintenance dosage for weight reduction is 5 mg, 10 mg or 15 mg once a week, and 15 mg is the maximum.

Why the label steps up slowly

The label gives the reason in one line: follow the escalation to reduce the risk of gastrointestinal adverse reactions. In the two main weight trials, nausea was reported by 25% of people on 5 mg, 29% on 10 mg and 28% on 15 mg, against 8% on placebo. Vomiting was 8%, 11% and 13%, against 2%. Diarrhea was 19%, 21% and 23%, against 8%.

The label adds that most nausea, vomiting and diarrhea happened during dose escalation and decreased over time. People in those trials reached their dose through an escalation period of up to 20 weeks.

What the label cannot tell you

The label’s side-effect numbers come from people who stepped up gradually. It has no data on starting at 10 mg, so it cannot say what to expect. It also cannot say whether your prescription was a mistake. That is a question for the prescriber who wrote it or the pharmacist who filled it, and a phone call answers it.

The Medication Guide says to stop using Zepbound and call your healthcare provider right away if you have severe stomach-area pain that will not go away, with or without nausea or vomiting.
