Short answer
The Zepbound single-dose pen instructions do not include a skin-cleaning step. They tell you to wash your hands, and they do not mention alcohol swabs. Wegovy’s pen instructions do say to clean the injection site, with an alcohol swab or soap and water. WHO’s injection guidance lists soap and water, not alcohol, for injections under the skin.
What the Zepbound instructions say
The FDA-approved Instructions for Use for the Zepbound single-dose pen, revised April 2026, run in this order: wash your hands, choose your injection site, pull off the gray base cap, place the clear base on your skin and unlock, then press and hold for up to 10 seconds. The words alcohol and clean do not appear in them.
The Zepbound vial and KwikPen instructions do list an alcohol swab as a supply. In those, the swab is for the vial’s rubber stopper or the KwikPen’s red inner seal. They contain no step for wiping the skin either.
What the Wegovy instructions say
Wegovy’s instructions are different. For the single-dose pen, the supplies list includes “1 alcohol swab or soap and water,” and the instructions say to clean the injection site with one or the other, not to touch the site after cleaning, and to allow the skin to dry before injecting. The Wegovy FlexTouch pen instructions say to wipe the skin with an alcohol swab and let the site dry before injecting. So the two brands’ official instructions do not match on this point. Each set applies only to its own product.
What WHO and CDC say
WHO’s best practices toolkit for injections, dated March 2010, has a table of skin preparation by injection type. For subcutaneous injections it lists soap and water as yes and 60–70% alcohol as no. That document is written for health care settings.
CDC’s vaccine administration chapter for clinicians, dated April 8, 2024, goes the other way for vaccines given under the skin. It says to cleanse the skin with a sterile alcohol swab and allow it to dry. Neither document is about injecting a weight-loss pen at home, and we found no CDC page that covers that.
If you skipped the wipe once
Nothing in the Zepbound pen instructions treats a skipped alcohol wipe as an error, because the wipe is not one of the steps. The instructions do say to wash your hands and not to touch the needle.
In the weight-loss trials, the label reports injection site reactions in 6% to 8% of people on Zepbound and 2% on placebo. It groups bruising, redness, itching, pain and rash under that term, and it does not link them to skin cleaning.
