How long do Zepbound side effects last?

Short answer

The FDA label does not give a number of days or weeks. It says most nausea, vomiting and diarrhea happened while the dose was being stepped up and decreased over time. In the trials, stepping up took up to 20 weeks. Stomach problems that are severe or will not go away are a reason to call your provider.

What the label says about timing

The label, revised in August 2026, says the majority of nausea, vomiting and diarrhea events occurred during dose escalation and decreased over time. In the two main weight trials, people reached their assigned dose over an escalation period of up to 20 weeks. The SURMOUNT-1 trial report adds that most stomach side effects were mild to moderate. The label also says most people who stopped Zepbound because of side effects did so in the first few months, because of stomach and bowel reactions.

What a pooled analysis of the trials added

A 2025 post hoc analysis by Lilly-affiliated researchers looked at the four SURMOUNT trials. It reported that stomach side effects occurred mainly during dose escalation, diminished over time and rarely led people to stop. When people needed medicine for nausea or diarrhea, the first use was most often during the first 24 weeks, which included the escalation period. The label also says the drug's slowing of stomach emptying is largest after the first dose and diminishes over time. Neither source gives a typical duration for one person.

What is not known

No source here says how many days nausea lasts after each shot, or how long side effects last after a dose increase. The label does not give a time course for constipation, fatigue or hair loss either. It links hair loss to weight reduction. These are averages from trials with a set step-up schedule. Your experience can differ, and the label's advice is that the dose escalation exists to reduce the risk of stomach side effects.

When to call your provider

The Medication Guide says to tell your provider if stomach problems are severe or will not go away, and right away if nausea, vomiting or diarrhea does not go away, because fluid loss can cause kidney problems. Call right away for upper stomach pain, yellow skin or eyes, fever or clay-colored stools, which can signal gallbladder problems. Stop Zepbound and call right away for severe stomach pain that will not go away, which can signal pancreatitis. Talk to your provider about any side effect that bothers you or does not go away.
