What the label reports
The label, revised in August 2026, pools two placebo-controlled weight trials of 2,519 adults treated for up to 72 weeks. Its table of common side effects lists fatigue, which counts asthenia, fatigue, lethargy and malaise together. Rates were 5% on 5 mg, 6% on 10 mg and 7% on 15 mg, against 3% on placebo. Fatigue is on the label's list of the most common side effects, reported in at least 5% of patients. The words sleepiness and somnolence do not appear as side effects anywhere in the label.
What the Medication Guide says
The Medication Guide, the leaflet written for patients, lists feeling tired among the most common side effects of Zepbound. It does not say when tiredness starts, how long it lasts or whether it fades. It says to talk to your healthcare provider about any side effect that bothers you or that does not go away. The label gives no time course for fatigue, so it cannot tell you whether feeling worn out a day or two after a shot is expected.
Label-listed causes worth ruling out
Low blood sugar is one. The Medication Guide lists drowsiness, weakness, dizziness and confusion among its signs, and says the risk may be higher with insulin or a sulfonylurea. In the type 2 diabetes trial, it was reported by 4.2% on Zepbound against 1.3% on placebo. Fluid loss is another. The guide says nausea, vomiting and diarrhea can cause dehydration. The label reports low blood pressure in 1.6% against 0.1%, more often with blood pressure medicines and alongside stomach side effects and dehydration.
Zepbound and sleep apnea
Zepbound is also approved to treat moderate to severe obstructive sleep apnea in adults with obesity. In those two 52-week trials, the label says people on Zepbound showed improvement in sleep-related impairment compared with placebo, measured with a standard patient questionnaire. Side effects in those trials were similar to the weight trials. If you snore or feel sleepy during the day, sleep apnea is worth raising with your provider whether or not you take Zepbound.
