Short answer
Because very little of it gets absorbed, and food or extra liquid lowers that further. The FDA label says to take one tablet each morning on an empty stomach with up to 4 ounces of plain water, then wait at least 30 minutes before eating, drinking or taking other pills. Only about 1% to 2% of the dose is absorbed.
What the label tells you to do
Wegovy tablets (oral semaglutide) were approved by the FDA for weight management on 22 December 2025. Section 2.1 of the prescribing information sets the routine: one tablet, once a day, in the morning, on an empty stomach, with water only, up to 4 ounces. No coffee, juice or other drinks with it.
Swallow it whole. The label says not to split, crush, chew or dissolve the tablet, and not to take more than one a day. Then wait at least 30 minutes before you eat, drink anything or take other oral medicines.
Why the rules are so strict
According to section 12.3 of the label, the tablet contains an absorption helper called SNAC (salcaprozate sodium), and the drug is absorbed mainly in the stomach. Even so, absolute bioavailability is only about 1% to 2%.
The label's own studies show why the details matter. Absorption was higher with a small sip of water (50 mL) than with a full glass (240 mL), and higher the longer people waited before eating. There was no meaningful difference between 50 mL and 120 mL, which is why the label allows up to 4 ounces.
Other pills, missed doses and storage
Because the tablet changes how other oral drugs are absorbed, the label notes that levothyroxine exposure rose 33% in an interaction study and advises monitoring other oral medicines. Tell your prescriber everything you take.
If you miss a dose, the label says to skip it and take the next one the following day. Store tablets in the original bottle at room temperature (68 to 77°F), away from moisture. This page summarizes the official label; follow your prescriber's instructions.
