What the trials show

A 2018 systematic review in The American Journal of Clinical Nutrition pooled 28 randomized trials with 1,924 people, each lasting at least 3 weeks. At a median dose of about 10.2 grams of psyllium a day, LDL cholesterol fell by 0.33 mmol/L, which is roughly 13 mg/dL. Non-HDL cholesterol and ApoB fell too.

The authors graded the LDL evidence as moderate quality, because results varied between trials. The effect showed up in people with and without high cholesterol. These trials measured cholesterol numbers, not heart attacks.

What the FDA allows on the label

FDA regulation 21 CFR 101.81 lets foods with psyllium husk carry a heart disease health claim. The daily intake the rule ties to that claim is 7 grams or more of soluble fiber from psyllium seed husk, as part of a diet low in saturated fat and cholesterol. A food must supply at least 1.7 grams of soluble fiber per serving to use it.

The wording matters. The claim must say the diet "may" or "might" reduce the risk of heart disease, and it cannot promise any set amount of risk reduction.

Is 250 to 500 mg enough?

Not by the numbers above. 500 mg is half a gram. The median dose in the LDL trials was about 10.2 grams, roughly 20 times more. The FDA claim figure is 7 grams of soluble fiber, 14 times more.

For scale, one over-the-counter psyllium powder lists about 3.4 grams of psyllium husk per dose on its Drug Facts label, taken up to 3 times a day. Nothing here shows that a few hundred milligrams does anything for LDL. If your LDL is high, psyllium is an add-on to discuss with your doctor, not a replacement for treatment.

Side effects and label warnings

The main warning is choking. FDA requires psyllium drug labels to say that taking it without enough fluid may cause it to swell and block your throat or esophagus. The label direction is to take each dose with at least 8 ounces, a full glass, of water or other fluid. Do not take it if you have difficulty swallowing.

Drug Facts labels also say to take psyllium 2 or more hours before or after other drugs, warn of allergic reactions in people sensitive to psyllium, and note you may get minor bloating or changes in bowel habits as your body adjusts.
