What these tests measure
Most consumer biological age tests read DNA methylation, chemical tags on your DNA that shift with age. A formula called an epigenetic clock turns those tags into an age or a pace of aging. The clocks were built to compare large groups in research, not to diagnose one person. DunedinPACE, one clock used by consumer brands, was published in 2022 in eLife as a research measure of the pace of aging, and its authors describe it as a tool for gerontology and geroscience. In those studies, a faster pace was linked with more illness, disability and death across groups of people.
The repeat-test problem
The biggest issue for you as a buyer is reliability. In the 2022 Nature Aging study by Higgins-Chen and colleagues, running the same DNA sample more than once gave results that differed by up to 9 years on six prominent clocks. That is technical noise, not real change in your body. The team built retrained versions that brought most repeat results within 1.5 years of each other. So if you test, retest and see your age drop, the change may be the lab, not your habits. Ask a company which clock it uses and whether it has published repeat-test data.
What the field itself says
A 2023 framework paper in Cell by aging researchers says the field still lacks agreed standards for what makes an aging biomarker reliable. It sets out validation steps that still need to happen before these tests can be used in clinical trials and everyday medicine. In practice, that means a biological age result is a curiosity, not a diagnosis. Standard measures your doctor already understands, such as ApoB, HbA1c and blood pressure, have clear targets and clear next steps. If you want a longevity baseline, those are the numbers with a track record behind them.
