Short answer
Through LillyDirect, Lilly’s own pharmacy, on 30 September 2026: $299 a month for 2.5 mg, $399 for 5 mg, and $449 for 7.5 mg and up if you refill within 45 days. Without that refill program the higher doses list at $499 to $699. Vials and the KwikPen cost the same. You still need a prescription, and vials need needles and syringes bought separately.
The maker’s self-pay prices
LillyDirect sells Zepbound single-dose vials and the multi-dose KwikPen at the same price since February 2026. A month, meaning 28 days, costs $299 at 2.5 mg, $399 at 5 mg, and $449 at 7.5 mg, 10 mg, 12.5 mg and 15 mg under Lilly’s Self Pay Journey Program, which requires a refill within 45 days of the last delivery. Miss that window and the regular price applies: $499 for 7.5 mg and $699 for the three highest doses.
With insurance
Lilly says people with commercial insurance that covers Zepbound may pay as little as $25 a month with its savings card, subject to terms. Since 1 July 2026, eligible Medicare Part D members pay a $50 copay under the Medicare GLP-1 Bridge. Government-plan members can’t use the commercial savings card.
What to watch
The cheapest dose is the starting dose; the label moves people up in 2.5 mg steps after at least four weeks each, so the monthly cost usually rises over the first few months. Compounded copies sold at lower prices are not FDA-approved, and the FDA acted against mass-marketed versions in 2026. Lilly’s pill, Foundayo, starts at $149 a month, and Wegovy’s intro price at NovoCare is $199, so there are legitimate cheaper routes to discuss with a prescriber.
